Add slot sequence numbers to the request

AFAIU dialogs follow the slot sequence. It would be nice to have the slot sequence numbers in the request for Elicit/ConfirmSlot customization.

    I am not sure if this idea would apply to you but in some of my skills, I assign a number to the slot's option "id" field. If applied to your issue, it could give you a sequential number for your slots.
